A very popular VPN, Windscribe, was seized by Ukrainian authorities, allowing all traffic that was captured at those specific servers during the time to be monitored and logged. This was due to a major flaw in some of the servers they were running; they were COMPLETELY unencrypted. Being one of the top choices for free "no log" VPNs, this is very likely to put a huge strain on trust between them and their customer base. However, they've been very transparent in the whole situation, taking complete responsibility and vowing to do better in the future, including the steps that will help them reach those goals.
Being a previous customer of theirs, this pretty much seals the deal for me that I will never be going back. Government seizures aren't exactly unheard of, but unencrypted servers? Big yikes.
